Dear MyCP Parent Partner,

Thank you for you time and commitment to helping Dompe with their upcoming drug trial.  Parent / family input is crucial to designing a successful clinical trial.

In order to get you paid, we need information about how you want to receive the funds.  The next steps are:

  1. W-9 [download], sign it and return it to ap@cprn.org. (You may have received this from Cori, our business manager, already.
  2. Fill in the form below (you must be logged into MyCP to see and complete the form) and submit it.
  3. Go to the Parent Advisor Invoice page to submit your invoice there (we will generate the invoice for you from that page).
  4. Get paid!
