A number of sites have recently sought to license CPCHILD and/or GOAL for their clinical practice.  Prices quoted for annual usage have varied but have been significant barriers to including this network established outcome measure in our shared practices and registry. We have approached SickKids to standardize a reduced license fee for these instruments through the network but as an addition to participation fees.  SickKids is proposing to standardize their clinical use license fees at $5,000 per instrument per year.  We are discussing a CPRN wide site license that would allow us to sublicense these instruments to CPRN sites for $1,000 per instrument per year (plus a 15% indirect cost).  In order to secure this substantial discount, the majority of our existing sites would need to agree to license one or both instruments through CPRN.  The survey below is to gather information about your sites usage, desired usage, implementation and licensing for CPCHILD and GOAL.

SickKids standard licensing is $5,000 per instrument per year. CPRN site licensing is proposed to be $1,000 per instrument per year with a 15% overhead charge.
