Please review the following session topics and make a selection for each one. Your choices will be used to determine which sessions are run and in what order to maximize attendees’ ability to attend the sessions most interesting to him/her/them. Each session is planned to be 90 minutes. Depending on your choices, you will be able to attend three to four breakout sessions so use your “Must attend” selections wisely. Sessions will only run once. Speakers, beware — please choose “Must attend” for your sessions! We will set final session times after everyone has voted on their preferred sessions. Each session description is followed by a required answer of how you rate your interest in the session. To advance the maturity of network, the Steering Committee has selected  items from multiple domains from the maturity grid  assessment to  improve the network and develop our next five year strategy.  The group process on this from 2021 and 2023 was invaluable to improving the maturity of our work as a learning health network.  This year, we want you to rank the topic based on your preference and then we will assign attendees as a function of preference and need to balance the breakouts.

The topics are:

  3. Environmental Scan for updated five year strategic plan

See the abstracts and the rubric for the three items chosen for improvement and the strategy development description.  The breakout group will work to make suggestions and prioritizations to advance our work in these areas. Title:Contributing data to the CP Research Network: how to optimize return on investment!

Session AbstractThe CP Research Network was established to improve the quality of life and health-related outcomes for individuals with CP and their families. To do so, high-quality collaborative clinical research is conducted using multi-center data that are contributed to CPRN’s data registry. The CPRN Registry undergoes continuous quality improvement where sites are working on the completeness and integrity of the data. CPRN and Data Coordinating Center (DCC) staff monitor data processes from acquisition through to harmonization, maintenance, and statistical analyses. By compiling a large, diverse dataset from clinical sites, the CPRN Registry enables investigators to collaboratively access comprehensive information for diagnosing comorbid conditions, determining prognostic factors, and evaluating clinical interventions. CPRN as an organization is committed to developing a core outcome set/family of measures that can be assessed regularly to track our maturity as a Learning Health Network. These data and analytics will be shared back to the sites and to investigators to use in their patient monitoring, QI, and Research. However, the opportunity for clinical sites to investigate the quality and utility of their own curated data has not been optimized. The intent of this session is to explore development of what Registry reports should be provided back to contributing sites for population management and quality improvement, and how sites can create a “local registry” of their own patients through EMR and REDCap reports. Attendees will be encouraged to share best practices across sites to understand how local (i.e., site-specific) reporting and a local registry using CPRN data could improve clinical care for patients with CP.
Objectives / Anticipated Outcomes:
  1. To learn the data curation and harmonization processes at CPRN
  2. Discuss the metrics that CPRN reports on a regular basis (e.g., a family of measures) to reflect our maturity as a Learning Health Network and initiatives to continuously improve

Title:Development of Leaders in the Network

Session Abstract:The CP Research Network has had an organic benefit of developing leaders in the field.  The combination of inspired, junior clinicians with experienced clinician researchers and clinical care leaders has aided in the development of multiple collaborative leaders in CP research and Quality Improvement (QI).  Study participation and leadership, QI discipline leadership, authorship and committee participation have given a variety of younger clinicians an opportunity to accelerate their careers.  Despite the organic leadership growth, its development has been ad hoc and less purposeful.

Given the value to both the network and the participating centers of experienced and developing leaders, this session will focus on how purposeful development can be done within the network in a way that complements and differentiates from other development opportunities offered by NIH K awards, AACPDM, mentorship at one’s home institution or other professional society avenues. The group will explore how organic leadership development has occurred within the network and determine if there are processes that can be formalized and replicated with a mix of volunteer and staff resources.   The group will compare and contrast these potential mechanisms with existing programs available to junior faculty and make recommendations as to how the network could consider improving it intentionality with the development of leaders.

Objectives/Anticipated Outcomes:
  1. Clear understanding of the differentiation between CPRN based development of leaders and other opportunities available to junior faculty.
  2. Recommendations as to the strategy the network should pursue to enhance its development of leaders.
  3. Understanding of the roles and responsibilities necessary to implement any proposed plan, as well as the resources required.

Title:Landscape Scan Affecting Cerebral Palsy Research and Care

Session Abstract:This breakout session aims to solicit insights from participants about the external landscape in the US regarding prospects for research and patient care for those interested in improving the health and well-being of people with Cerebral Palsy. Using the PESTLE framework, participants will assess Political, Economic, Social, Technological, Legal, and Environmental factors influencing these prospects. CPRN will use the results of these sessions to ground strategic planning exercises during the remainder of 2025.

  1. Ground the CPRN strategic planning process in a crowd-sourced view of the various factors external to CPRN that may influence CPRN’s strategy.
  2. Engage participants in big-picture thinking about major factors CPRN should be considering as it sets its future strategy.

Title:Mind the Gap: Best practices for following and intervening with kids that are at risk for CP

Session Abstract:How do the youngest kids enter your CP clinic? How can you support earlier diagnosis and intervention for kids at risk for CP? Guidelines have rapidly transformed recommended practice for early detection of cerebral palsy even in more mild cases (Novak et al, 2017). Child neurologists and neurodevelopmental providers play a key role in diagnosing and managing CP (Aravamuthan et al, 2020). While CP previously was diagnosed between ages 2-3 in the U.S., earlier diagnosis is often possible, particularly in more severe cases.

The early diagnosis of CP in infants < 2 years of age is crucial for optimizing outcomes and guiding appropriate interventions (Maitre et al, 2020) but implementing the Early Diagnosis guidelines can be very challenging. This session will provide a review of the literature that identifies the key tools for identifying early signs of CP include the Hammersmith Infant Neurological Examination (HINE), General Movements Assessment (GMA), functional assessments, imaging, and a comprehensive patient history with a focus on the essential role of critical timing and use of serial evaluations to monitor developmental milestones in early identification. We will discuss clinical implementation in a variety of practice environments.

Mind the Gap Session Interest

Title:Natural History Studies and the CPRN Registry

Session Abstract:The Cerebral Palsy Research Network (CPRN) infrastructure and clinical registry offers a unique opportunity to conduct studies to evaluate the natural history of a cerebral palsy population. This was evident in the discussion during the 2024 meeting of the Cerebral Palsy Research Network when, across multiple sessions, the idea of conducting natural history studies was raised.

During the course of this presentation, first work to define the concept of a natural history study. This will be followed by a summary on natural history studies of cerebral palsy cohorts available in the scientific literature. This will include reports on hip development, gross motor function, and scoliosis. Next, there will be a brief review of tools that are available to aid in the conduct of natural history studies, specifically the information available in the NIH toolkit including general structure of natural history studies and funding opportunities for natural history studies. Lastly, we will share some longitudinal data available in the CPRN registry and brainstorm ideas for data analysis and grant submissions around to natural history them.

Title:Capturing Orthopedic Surgery Case Information

Session Abstract: The orthopedic surgical care of children with Cerebral Palsy involves a wide menu of different surgical procedures that are indicated at different ages and different degrees of musculoskeletal impairment. The variety of surgical options presents many challenges to determining best practice guidelines, which includes wide practice variations, multiple combinations of the variety of surgical options of which to choose, and the multiple variables that can impact surgical outcomes. In addition, the burden of entering additional surgical data forms can be a challenge for busy pediatric orthopedic surgical practices. The breadth of centers involved in CPRN, however, gives the opportunity to leverage those different perspectives to make strides for improved orthopedic care.   However, those improvements will need dedication, commitment, and investment from the orthopedic surgeons in CPRN, which gives rise potentially for the need for a CPRN Orthopedic Study Group.

Title:An Ounce of Prevention: The Importance of Primary and Preventive Care for Adults with Cerebral Palsy

Session Abstract: Adults with cerebral palsy (CP) are at higher risk for many chronic conditions, and at younger ages, compared to the general population. Primary care and preventive screening can help identify many of these chronic conditions at earlier stages, when they are more amenable to treatment. Additionally, a study involving multiple stakeholders identified primary and preventive care as a top priority for the adult CP research agenda. This session will focus on the role of primary and preventive care for adults with CP. We will discuss recently completed, ongoing, and planned projects through the Cerebral Palsy Research Network (CPRN) focused on primary care. In addition, we will explore the role of the primary care provider (PCP) in the care of adults with CP, the importance of preventive care, and barriers to comprehensive preventive care and preventive screening. Below is a detailed outline and specific content to be covered within this session.

  1. Review results from CPRN Accelerator Award study focused on adult CP research priorities (Cristina; 10 min)
  2. Share preliminary results from CPRN‚adult healthcare capacity survey (Ed; 10-15 min)

Adult Primary and Preventive Care Session Interest

Title:Facilitating knowledge to action: the value of the Cerebral Palsy Research Network for co-producing research from idea generation through implementing new evidence

Session Abstract:Pain and functional changes with aging are a research priority and increased participation by adults with cerebral palsy in the community registry surveys can help advance efforts.  This course will provide investigators with an updated report on the enrollment and initial findings of the surveys about functional changes with aging and pain among adults with cerebral palsy.  Seed funding has allowed for evaluating different methods of recruitment, through social media with collaboration with Cerebral Palsy Foundation and through clinic lists at the University of Michigan.  Process for institutional review board determination of study as “recruitment only” will be discussed as a potential strategy at other CP Research Network sites. Additional strategies for recruitment will be discussed along with aligning with community agencies and organizations that serve adults with cerebral palsy. Additional sources of funding will be explored, as well as other methods to increase engagement of adults with cerebral palsy in the registry.  Initial findings of the registry will be reviewed for both functional changes and pain experiences.  Functional changes with age and across different levels of gross motor, manual, and communication abilities will be described. Common experiences with pain location, interference, and treatments will be reviewed. Opportunities for collaboration on secondary analysis will discussed.  Next steps for pain assessment and intervention studies will be explored. The session will be interactive, and allow for discussion, brainstorming, collaboration, and project development.
Objectives/Anticipated outcomes1. Learn how clinicians, researchers, and community members collaborate to identify a clinical concern such as adult pain, and develop a clinical research question via the following:
a. Selecting an existing research topic developed through a Research CP prioritization effort
b. Discussions in the MyCP community forum

2. Learn how to cultivate or leverage an existing CPRN working group and identify a leader who will begin plans for developing research methodology around a particular topic, including utilizing existing data in CPRN clinical and/or community registries.

Facilitating knowledge to action Session Interest

Title:Neurosurgery for hypertonia

Session Abstract:Intrathecal baclofen (ITB) therapy is a common management strategy for medically refractory hypertonia in patients with cerebral palsy (CP). Chronic use of ITB exposes patients to risks of sudden baclofen withdrawal if the system fails which can, in its most severe form, become fatal. Sudden withdrawal may be precipitated by noninfectious complications. Noninfectious complication rates remain unknown with retrospective series unable to identify causative modifiers. Understanding the true incidence of catheter revisions and modifiers across this learning health network is directly relevant to the CP community and stakeholders will help guide collaborative efforts to optimize surgical care. We reviewed the prospective data entered into the CPRN registry from beginning to December 2024 for all patients with CP who underwent an index ITB pump system implantation and was then reoperated for any reason besides infection. We categorized the reasons for reoperation and intend to develop Kaplan-Meier curves defining survivability of pump and catheter systems. We will also look for associated or causative modifiers of noninfectious complications. The study will be ready for analysis at the annual meeting. Neena will discuss the status of her project and the analysis involved. Jeff Leonard will give an update on Focal 2.

Title:Improving hip health across the CP Research Network

Session Abstract:The American Academy of Cerebral Palsy and Developmental Medicine (AACPDM) has a care pathway entitled‚ Hip Surveillance in Cerebral Palsy that describes by age, GMFCS and Winters Gage Hicks gait type the process of monitoring and identifying critical early indicators of hip displacement.

In 2021 CPRN administered a survey to determine the status of hip surveillance practices across network sites. Only 14% reported a standardized process. These efforts kicked off the Hip surveillance QI work in which we engage a community of practice of interested CPRN sites.

Our initial smart aim as a network was to improve hip screening for individuals classified as GMFCS IV and V. We learned quickly that the first step in the process to improve was documenting the GMFCS level as a discrete element. As of March 2025, we have 4 sites currently working on improving hip surveillance at their site.

In this session we will first describe an overview of the process mapped for surveilling hips and demonstrate QI tools to organize our improvement work. We will describe the CPRN hip surveillance bundle (hip x-rays ordered, hip x-rays obtained, migration percentage documented) that sites are encouraged to collect. Then, a presenter from each of the four sites will share best practices used at their institution and interventions they employed to increase bundle completion; Lastly, we will facilitate a discussion among the participants as to current practices and efforts across sites and how to engage in the CPRN hip surveillance work.

Objectives/Anticipated outcomes1. Participants will learn about tools used in the QI process including Key Driver Diagram and Failure Effects Mode Analysis and how they are being implemented at network sites to improve hip surveillance.
2. Participants will be able to identify the CPRN Hip surveillance bundle elements.
3. Participants will understand best practices shared by network sites and gain insight through discussion as to how they could be employed at their site.

Improving hip health across the CP Research Network Session Interest

Title:New opportunities in CP genetics

Session Abstract: About one in three individuals with cerebral palsy have an identifiable genetic cause for their condition at the present time, while fewer than 10% of CP cases are believed to be a direct result of perinatal birth injury. Advances in CP genetics are beginning to impact diagnosis and treatment options for patients and families. In this breakout session, we will review the state of the science in CP genetics and discuss current genetics initiatives within the CPRN as well as opportunities for ongoing collaboration. We will share recent technological advances in genome sequencing and analysis, including the value of RNA sequencing combined with genomics as well as the potential utility of long read whole genome sequencing to identify cryptic mutations and to provide new insights in epigenomics relevant to CP. We will review treatment implications for CP based on genetic findings, including the clinical actionability of genetic forms of CP as well as gene-targeting therapies under development. Finally, we will generate ideas for strengthening CP genetics projects within the CPRN and discuss new opportunities for partnership with industry and patient advocacy leaders within the rare disease community. Opportunities for early stage investigators, and those new to CP genetics will be emphasized.

Title:Gaining Traction with the Adult Clinical and Community Registry

Session Abstract: In this session, we will review the CPRN Community and Clinical registries regarding actionable items that advance knowledge and science about the clinical management of adults with cerebral palsy (CP). The session will begin by reviewing the community registry enrollment, highlighting areas for improving the engagement of specific populations with annual and follow-up surveys. We will brainstorm ways to minimize respondent burden and increase the ability to answer more specific research questions.  We will review findings from the initial analysis of the existing 800+ respondents regarding functional decline and pain and generate hypotheses and/or additional research questions. We will highlight available information in the clinical registry concerning adults with CP. Finally, we will brainstorm potential ideas for projects with adults within the network surrounding pain management and functional decline.
Objectives/Anticipated Outcomes1. Develop an action plan for creating a proposal for adjustments to the Adult Community Registry

Gaining Traction with the Adult Clinical and Community Registry Session Interest

Title:Determining the Impact of Dystonia Screening

Session Abstract:Dystonia is a common, debilitating, but underdiagnosed symptom of CP. The Dystonia QI Workgroup for CPRN has demonstrated that using a 5-item screening algorithm as a part of CP clinic visits can increase the percentage of children with CP diagnosed with dystonia from 52% to 69% across three CPRN sites. An electronic medical record documentation template was used for the original implementation of this screening algorithm. Further implementation options now including the CPRN smartforms. In this session, we will describe our plan to assess the impact of dystonia screening across CPRN sites by randomizing interested sites to step-wise implementation. In line with the CPRN Community Driven Research Agenda for Dystonia in CP, measurements of impact will include: 1) Patient Global Impression of Improvement after implementation of screening, 2) Patient knowledge of whether they have dystonia, and 3) Tone medications and dosages prescribed at clinic visits. Sites participating in this step-wise implementation can be considered as study sites in an upcoming PCORI funding submission.
Objectives/Anticipated Outcomes1. Describe the 5-item dystonia screening algorithm that has increased dystonia diagnosis rates across three CPRN sites.
2. Describe the planned PCORI funding submission to assess the impact of dystonia screening.

Session Abstract:In this breakout session, we will learn more about the programs and support provided by the National Center on Health, Physical Activity and Disability (NCHPAD). NCHPAD is a public health practice center dedicated to promoting health and well-being of people with disabilities through evidence-based programs and resources. At the core of NCHPAD‚ offerings is NCHPAD Connect, a participant portal that plays a vital role in advancing health promotion efforts. This online portal provides access to a growing community of health promotion programs, resources, and experts for people with disabilities at no cost. NCHPAD Connect is designed to support the personal health goals and well-being of people with mobility limitations aged 18 and older. We will provide more depth information about the resources available at NCHPAD to members of the Cerebral Palsy Research Network (CPRN). We will brainstorm strategies for adapting the programs and resources for children and explore strategies for creating connections between CPRN clinics, NCHPAD, and local programs. Work groups and action plans will be created.
