This CDE captures data from SF-36 version 1.0, with permission from the RAND Corporation. Scoring and validation differ for editions by the RAND Corporation (http://www.rand.org) and by the National Multiple Sclerosis Society (NMSS, http://www.nationalmssociety.org) for Multiple Sclerosis Quality of Life instruments. Questions and values for SF-36 version 2.0 differ from version 1.0. Separate permission must be obtained from QualityMetric Inc. (http://www.sf-36.org) to use that instrument.
